CCC offers health care for all your needs in a single place, and our providers see you as a whole person. Search below for the service you need.

COVID-19 note: During the pandemic we are offering appointments by phone and video in addition to in-person visits.  We also offer COVID-19 testing and vaccines.

Primary health care

Your primary care provider can treat you if you get sick, help you manage chronic illnesses, help you prevent illness and manage addiction, answer questions and connect you to more treatment options.

On-site pharmacy

There’s no need to go elsewhere for the medications your CCC provider prescribes. It’s easy to get your prescription filled and talk to a pharmacist if you have questions after your appointment.

Recovery

You have choices in your recovery. CCC values all pathways to recovery, from abstinence-based to medication-supported approaches. Our services include substance use treatment through primary care, detoxification services, medications like Suboxone that reduce withdrawal symptoms, recovery groups, one-on-one counseling and peer support.

Mental health

Our mental health providers offer counseling, therapy and medication to support your emotional wellbeing or help in a crisis. Our speciality mental health programs (Old Town Recovery Center, Puentes and Imani Center) offer a higher level of support from a mental health specialist. More limited mental health services are available through primary care at our clinics (Blackburn Center and Old Town Clinic).

Acupuncture

CCC offers acupuncture to help you manage chronic pain, addiction recovery, depression and anxiety, or to support your overall wellness.

Hep C treatment

We make it simple for you to treat hepatitis C with medication. Treatment takes about two months and has a cure rate of more than 95%. Learn more about hep C testing and treatment here.

Culturally specific services

For clients who are Black, African American and Latinx/Hispanic, we offer mental health care and addiction treatment in a setting that supports your culture and lived experience.

Recuperative care

We offer respite care for people who have nowhere to go after being discharged from the hospital. Recuperative care is available by referral only.

Charlette's Story

Homeless and on heroin, Charlette needed a fresh start. But she didn’t expect much when she landed at CCC’s Blackburn Center. That all changed in a matter of hours. With access to addiction treatment, primary care, a safe place to live and peers to support her — all under one roof — Charlette’s transition into recovery was smoother than she imagined. Having housing and health care in the same place is “huge,” she says. “You saw me — all of me — within the first day.”
