Primary care and routine health screenings
Managing chronic health conditions
Minor medical procedures
Mental health counseling, medication, individual and group therapy or referral to a specialist if you need more support
Addiction/substance use treatment, including medication-supported recovery with Suboxone (buprenorphine)
Consultation with a pharmacist about your prescription
On-site medication refills

Consumer drug take-back (open to CCC patients as well as the general public)

Wednesday, 9:30 am to 5:30 pm

Our on-site pharmacy makes it easy to get your CCC prescriptions filled. Our knowledgeable pharmacists can answer any questions you have about your medications.

How to Access Care

We offer in-person, phone and video appointments. Call (971) 361-7700 to schedule a visit.

Insurance Information

We don’t turn anyone away for lack of insurance or inability to pay. We accept Oregon Health Plan, Medicare, self-pay and some private insurance. Need coverage? We can help you get set up.

Charlette's Story

Homeless and on heroin, Charlette needed a fresh start. But she didn’t expect much when she landed at CCC’s Blackburn Center. That all changed in a matter of hours. With access to addiction treatment, primary care, a safe place to live and peers to support her — all under one roof — Charlette’s transition into recovery was smoother than she imagined. Having housing and health care in the same place is “huge,” she says. “You saw me — all of me — within the first day.”
