Access to health care can be life-changing—especially for people who are healing from illness, homelessness, or addiction. At CCC, our Primary Care, Pharmacy, and Recuperative Care Program (RCP) teams work together to help people stabilize and move forward.
Integrated Care Supporting Healing and Stability
CCC offers a wide range of health services that include primary care and pharmacy, to support the whole person in two locations: the Blackburn Center, on the east side and Old Town Clinic, in downtown Portland. Patients can receive routine health screenings, help with managing chronic conditions, and minor medical procedures. The team also provides mental health counseling, individual and group therapy, and referrals to specialists when more support is needed.
For people working toward recovery, CCC offers addiction treatment, including medication-supported recovery with Suboxone (buprenorphine). Patients can also meet with a pharmacist to talk about their prescriptions and refill medications on-site. Additional services include Hepatitis C treatment, acupuncture, and a consumer drug take-back program open to both CCC patients and the public.
Working alongside them is the Recuperative Care Program (RCP), which offers a safe place for people to heal after leaving the hospital. Located in CCC’s Evergreen Crossing building, RCP provides medical recuperation services for low-income and homeless individuals who have received medical care through a hospital system, providing essential continuing and proactive support for effective recovery. With 84 available beds, RCP fulfills our pledge to increase the number of beds open to those experiencing care gaps and reduces the strain on our health care system. Additionally, the Evergreen Crossing Primary Care Clinic treats clients located in RCP.
Instead of being discharged back to the streets, patients can stay at RCP for up to three months—or longer, depending on their medical needs. During that time, they receive care, support, and help getting connected to housing and other services.
By providing access to health care, immediate placement in transitional housing and intensive case management services, RCP improves health outcomes for at-risk patients, improves the efficiency of participating hospitals and prevents patients from being discharged into shelters or onto the street.
Together, our primary care and RCP programs are a key part of CCC’s larger health care system, helping patients move from crisis to stability.
Since its inception in 2005, RCP has served thousands of individuals, with over 65% resolving their acute medical condition, over 90% engaging with a primary care provider upon program exit and over 60% transitioning into stable housing. In 2025, CCC offered primary care services to 9,932 patients, and an additional 1,200 patients were served through our Mobile Health services.
Creating Pathways from Crisis to Stability
Healing is hard without a safe place to rest. The streets are not designed for recovery. RCP gives patients time and support to stabilize, while primary care and pharmacy teams make sure their health needs are fully addressed.
The impact goes far beyond medical care. Staff have seen patients move from homelessness and serious health challenges to stable housing, employment, and meaningful relationships. Many even return to give back to the community. By meeting urgent health needs and connecting people to long-term support, these teams help individuals become the best version of themselves.
