Eligible applicants who are engaged with self-sufficiency and case management programs can receive up to two preference points for higher priority on the Hazel Heights waitlists. Both of the following forms are optional and may be completed by programs on behalf of an applicant.
- Form 4d1 – For waitlist applicants who have successfully completed or are participating in a self-sufficiency program.
- Form 4d2 – For waitlist applicants who will receive ongoing case management after move-in.
