Hazel Heights is a 153-unit Low Income Housing Tax Credit building located in the Hazelwood neighborhood of SE Portland. Opened in 2018, it is conveniently located near bus and MAX lines with one- and two-bedroom units available.

The fair market (no rent subsidy) 1 and 2 bedroom waitlists is open as of 9/19/2024.

The waitlist for fair market one- and two-bedroom units are currently open.  Please note that the units do not have rent subsidy attached- applicants must meet a minimum income requirement and should anticipate being fully rent responsible.  Please refer to the Hazel Heights Income Flyer  for more info. Pre-Applying to Hazel Heights maroon arrow pointing right

  • Applicants must submit the Pre-Application Form in order to be considered for the Hazel Heights waitlist. CCC Housing can only accept pre-application forms that are complete and signed.
  • Applicants must initial on the Pre-Application Form that they have read and understand the Hazel Heights Building Criteria.
  • Eligible applicants who are engaged with self-sufficiency and case management programs can receive up to two preference points for higher priority on the Hazel Heights waitlist.
  • Form 4d1 is for waitlist applicants who have successfully completed or are participating in a self-sufficiency program. This form is optional and may be completed by programs on behalf of an applicant.
  • Form 4d2 is for waitlist applicants who will receive ongoing case management after move-in. This form is optional and may be completed by programs on behalf of an applicant.

You have two options for submitting your pre-application form:

  • Email. Once the waitlist is open send your completed and signed Pre-Application Form to hazelheightsapply@ccconcern.org.
  • In-person. Once the waitlist is open bring your completed and signed Pre-Application Form to the CCC Housing Office at 523 NW Everett St., Portland OR, 97209. Application intake will be held outside, following physical distancing protocols.

Note: Due to the ongoing pandemic, we strongly encourage applicants to submit pre-application forms by email to hazelheightsapply@ccconcern.org. CCC will not accept faxed applications or those submitted directly at Hazel Heights.

Pets are not allowed at Hazel Heights.  Applicants/Tenants may request service or assistance animals as a reasonable accommodation when necessary to support a verified disability. All requests are subject to management approval.

Eligibility and screening

  • A security deposit is not required at Hazel Heights.
  • The updated Hazel Heights Building Criteria establishes new criminal screening and look-back periods. See sections 6a and 7b of the Hazel Height Building Criteria for more information.

Units at Hazel Heights are income and rent restricted based on a percentage of Median Family Income (%MFI) for Multnomah County. Households must earn annual income below the corresponding %MFI to be eligible for the unit. In addition, when applicants are contacted for intake they will be required to demonstrate their monthly household income is two times the monthly rent.
