If a waitlist is open, first review the screening and eligibility requirements listed in the Building Criteria.

Only submit a pre-application if you have checked and meet these requirements. If you do, submit your completed and signed pre-application materials. This includes:

•  Blackburn FM Waitlist Pre-Application (required)
• Partner Agency Referral Form (optional)
  • This is not required; however, applicants who submit this completed and signed form from a program listed on the Partner Agency List will receive preference on the waitlist.

You can submit your materials in one of the following ways:

• In-person: Drop off at the CCC Housing Office, 523 NW Everett St., Portland OR, 97209.

Note: CCC strongly encourages applicants to submit pre-application forms by email so there is a electronic record and we can provide a receipt. CCC will not accept faxed applications or those submitted directly at the Blackburn Building.
