Summit Team: Serving Our Most Complex Patients

CCC’s Summit team cares for our most complex and medically fragile patients at Clínica del casco antiguo. Instead of expecting these patients to fit into a one-size-fits-all health care system, the Summit team adapts the system to work for them. Summit offers flexible scheduling, around-the-clock availability and even home visits.

Like many CCC programs, the Summit team goes above and beyond to break barriers. They work to narrow gaps that can keep vulnerable individuals from becoming as well and healthy as they can be.
